What is the Unit About?
Unit Description
The unit enables students to examine the foundation of integrated care in health and across Australian communities. There is a specific focus on investigating inter-professional collaboration and practice for the delivery of whole of person care. Students will examine and evaluate integrated care models and their suitability for different healthcare contexts. Students will critically appraise integrated care KPI’s to achieve efficient and effective healthcare services. Students will have opportunities to hear from experts across Australia and participate in industry case studies and analysis.

As per the Assessment and Results Policy 1.3, your results will reflect your achievement against specified learning outcomes.
On completion of this unit, you will be able to:
1.
Analyse theories that guide practices of integrated care in the delivery of health services across Australian communities.
2.
Identify, evaluate and prioritise integrated care models for healthcare settings of different contexts.
3.
Assess and improve KPIs for integrated care from the perspective of different stakeholders.
4.
Appraise the management challenges and opportunities associated within inter-professional collaborative practice across the continuum of care.
5.
Judge future practices, theories and challenges for integrated care within the whole systems perspective.

Assessment Task 1: Critical Argument Presentation
Task Description:
An integrated care strategy needs to be underpinned by principles of integrated care (for example, person centred, shared information, and continually improved). This assessment aims for you to explain an aged care or health organisation’s integrated care strategy. Particularly, how the strategy generates value for relevant stakeholders by being underpinned with specific principles of integrated care. In this assessment, assume yourself to be working in an Aged Care or other healthcare organisation and submit the assessment in a power-point presentation (with embedded voice).
Task Length:
A power-point presentation of 13 minutes with embedded continuous voice recording. Presentation time is allowed to vary by +/- 10%.

Assessment Task 2: Critical Analysis Report
Task Description:
Deciding on appropriate key performance indicators (KPIs) in an integrated care initiative is a complex task. This assessment aims for you to conduct a critical analysis of the use of performance measures to guide an integrated care strategy in aged care or other health organisation. In this assessment, assume yourself to be working in an Aged Care or other healthcare organisation, and submit the assessment in a report format.
Task Length:
3000 words (10% +/- word count). Any other variance in word count will attract a penalty. The word count includes such items as headings, quotes, in-text citations, and the executive summary. It does not include the table of contents, any appendices, and reference list at the end of the assignment.

Assessment Task 3: Evaluation Report
Task Description:
Integrated care initiatives can benefit from guidance of Systems thinking. This assessment aims for you to evaluate a substantial local integrated care initiative in your aged care or other healthcare organisation (or a substantial international integrated care initiative) through the lens of a whole of system perspective or systems thinking. In this assignment, the focus is on a specific aspect of systems thinking i.e., the importance of the interrelations among the components in a complex situation. You are to submit the evaluation of the chosen integrated care initiative in a report format.
Task Length:
3800 words (10% +/- word count). Any other variance in word count will attract a penalty. The word count includes such items as headings, quotes, in-text citations, and the executive summary. It does not include the table of contents, any appendices, and reference list at the end of the assignment.

How your final result is determined
To pass this unit, you need to demonstrate your attainment of each of the Intended Learning Outcomes, achieve a final unit grade of 50% or greater, and pass any hurdle tasks.
Submission of assignments
Where practicable, assignments should be submitted to an assignment submission folder in MYLO. You must submit assignments by the due date or receive a penalty (unless an extension of time has been approved by the Unit Coordinator). Students submitting any assignment in hard copy, or because of a practicum finalisation, must attach a student cover sheet and signed declaration for the submission to be accepted for marking.

Requests for extensions
If you are unable to submit an assessment task by the due date, you should apply for an extension.
 
A request for an extension should first be discussed with your Unit Coordinator or teaching support team where possible. A request for an extension must be submitted by the assessment due date, except where you can provide evidence it was not possible to do so. Typically, an application for an extension will be supported by documentary evidence: however, where it is not possible for you to provide evidence please contact your Unit Coordinator.
 
The Unit Coordinator must notify you of the outcome of an extension request within 3 working days of receiving the request.
Late penalties
Assignments submitted after the deadline will receive a late penalty of 5% of the original available mark for each calendar day (or part day) that the assignment is late. Late submissions will not be accepted more than 10 calendar days after the due date, or after assignments have been returned to other students on a scheduled date, whichever occurs first. Review of results and appeals
You are entitled to ask for a review of the marking and grading of your assessment task if there is an irregularity in the marking standards or an error in the process for determining the outcome of an assessment. There is no prescribed text or software for this unit. The learning materials of the unit will be drawn from publications, journal articles and other credible sources/websites. You will be able to access the learning materials through the Learning Content section of the MyLO unit. Journal articles and other readings will be made available throughout the semester via MyLO.
