What is the Unit About?
Unit Description
 
The unit provides students with an opportunity to develop the skills and capacity to apply integrated care in healthcare contexts. Students will draw on theories, models and tools from the literature of integrated care and assess their application in the delivery of complex healthcare services. Students will gain the skill to investigate the constraints and opportunities for partnership with patients and family, interprofessional practices and whole person care. Students will evaluate organisational resources, systems and value proposition for effective integrated care delivery. Students will have opportunities to hear from experts across Australia and participate in industry case studies and analysis.

As per the Assessment and Results Policy 1.3, your results will reflect your achievement against specified learning outcomes.
On completion of this unit, you will be able to:
1.
Analyse stakeholder needs, whole person care and communication requirements for effective implementation of integrated care in healthcare.
2.
Compare industry practices for integrated care nationally and internationally, within the changing environment.
3.
Critique application of integrated care, examining the requirements of interprofessional practices across the continuum of care and different sectors.
4.
Justify application of integrated care models for partnership with patients and family in healthcare.
5.
Appraise strategies for monitoring and evaluating KPIs of integrated care for value proposition in healthcare.

Assessment Task 1: Poster Presentation
Task Description:
The leadership team in your health organisation requires you to plan a campaign, which communicates an inspirational message regarding how staffs (clinicians and health management professionals in non-clinical area or health
managers) facilitate the effective implementation of integrated care.

This campaign should draw on lessons learnt from functioning of integrated care initiatives in the past. The campaign is to be compiled in a poster for the clinicians and health managers and the rationale of the campaign is to be presented to the senior management in your health organisation.
Task Length:
Part 1: A power-point presentation of 12-13 minutes, 14-15 slides, with embedded continuous voice recording. . Part 2: A poster in the dimension of width 59.4cm x height 84.1cm in one power-point slide. Do not include a voice recording or references on the poster slide. Submission should be made in pptx file format.

Assessment Task 2: Report
Task Description:
You are a manager in a health organisation (e.g., local health district, primary health network), leading the implementation of an integrated care project. The targeted cohort for this project
is people aged 65 and over (or 50 years or older Aboriginal or Torres Strait Islanders), who are experiencing a combination of mental and physical chronic conditions. Prepare a report,
explaining the value proposition of a collaborative partnership that the project would require between your organisation (the lead agency) and an aged care or General Practitioner facility (Government funded) in the community. You must also discuss
the required leadership and change management abilities to support inter-professional practices in this partnership. Finally, you are to engage in a discussion whether including a private
health or social services organisation as an additional partner could enhance the value proposition of the partnership.
Task Length:
3000 words (10% +/- word count). Any other variance in word count will attract a penalty. The word count includes such items as headings, quotes, and in-text citations. It does not include the title page, table of contents, any appendices, and reference list at the end of the assignment.

Assessment Task 3: Case Study
Task Description:
Investigate the issues of partnership with patients and application of model of integrated care for a specific healthcare initiative i.e. Commonwealth integrated care program “Health Care Homes”. Your task is to compile a case study, critically analysing the functioning of the program, so that you can plan to offer an improved home supported services for older clients in aged care or general clients who are accessing home supported services.

How your final result is determined
To pass this unit, you need to demonstrate your attainment of each of the Intended Learning Outcomes, achieve a final unit grade of 50% or greater, and pass any hurdle tasks.
Submission of assignments
Where practicable, assignments should be submitted to an assignment submission folder in MYLO. You must submit assignments by the due date or receive a penalty (unless an extension of time has been approved by the Unit Coordinator). Students submitting any assignment in hard copy, or because of a practicum finalisation, must attach a student cover sheet and signed declaration for the submission to be accepted for marking.

Requests for extensions
If you are unable to submit an assessment task by the due date, you should apply for an extension.
 
A request for an extension should first be discussed with your Unit Coordinator or teaching support team where possible. A request for an extension must be submitted by the assessment due date, except where you can provide evidence it was not possible to do so. Typically, an application for an extension will be supported by documentary evidence: however, where it is not possible for you to provide evidence please contact your Unit Coordinator.
 
The Unit Coordinator must notify you of the outcome of an extension request within 3 working days of receiving the request.
Late penalties
Assignments submitted after the deadline will receive a late penalty of 5% of the original available mark for each calendar day (or part day) that the assignment is late. Late submissions will not be accepted more than 10 calendar days after the due date, or after assignments have been returned to other students on a scheduled date, whichever occurs first. Review of results and appeals
You are entitled to ask for a review of the marking and grading of your assessment task if there is an irregularity in the marking standards or an error in the process for determining the outcome of an assessment.
