What is the Unit About?
Unit Description
 

In this unit, you will explore and compare different models of service design, articulation and delivery and how they influence health and wellbeing outcomes for diverse population groups. With a focus on rights, self-determination and strengths-based approaches, you will investigate the key principles of consumer engagement and participation, person-centred partnerships and co-design. You will explore the concepts of wellness, empowerment, reablement, and recovery and how they can be used to support individuals and families to achieve meaningful, purpose-driven outcomes.

As per the Assessment and Results Policy 1.3, your results will reflect your achievement against specified learning outcomes.
On completion of this unit, you will be able to:
1.
Evaluate different models of service design and delivery to inform quality personalised support.
2.
Communicate and use person-centred and strengths-based approaches to achieve purpose-driven outcomes for individuals and their support networks.
3.
Apply engagement, partnership and co-design principles and skills to facilitate planning and decision making for personalised support.

Assessment Task 1: AT1: Strengths Assessment and Reflection
Task Description:
For this task, you will draw from Weeks 1, 2 and 3 MyLO content, including the scenarios and examples of contemporary support, to respond to a series of questions related to person-centred strengths-based practice.

A dedicated AT1 page will be provided to you in MyLO for further guidance.

Assessment Task 2: AT2: Co-design Interview
Task Description:
You will be required to form pairs for this task. The task requires you to apply your interpersonal and communication skills to engage with a simulated client (your partner); collect information about their situation and support needs, and codesign a strategy for one identified need. This engagement will occur in an online or face-to-face environment.
You will then prepare a short reflection on the interview experience. This assessment (interview and reflection) is to be recorded and submitted to MyLO for marking. This is an individual assessment task (you will be assessed on your performance as the support professional). However, you will also support your partner by assuming the role of the client for their recording.

Further information and instructions for this task will be provided via MyLO.

Assessment Task 3: AT3: Case Study
Task Description:
For this task, you will be required to undertake person-centred planning and the design of individualised support. This will be based on a client case study.

The process will culminate in a written report based on guidelines provided via MyLO.

You will be expected to engage with relevant practice principles, frameworks and scholarly literature and evidence to help contextualise and justify the information presented in the report.

How your final result is determined
To pass this unit, you need to demonstrate your attainment of each of the Intended Learning Outcomes, achieve a final unit grade of 50% or greater, and pass any hurdle tasks.
 
Submission of assignments
Where practicable, assignments should be submitted to an assignment submission folder in MYLO. You must submit assignments by the due date or receive a penalty (unless an extension of time has been approved by the Unit Coordinator). Students submitting any assignment in hard copy, or because of a practicum finalisation, must attach a student cover sheet and signed declaration for the submission to be accepted for marking.
 
Requests for extensions
If you are unable to submit an assessment task by the due date, you should apply for an extension.
 
A request for an extension should first be discussed with your Unit Coordinator or teaching support team where possible. A request for an extension must be submitted by the assessment due date, except where you can provide evidence it was not possible to do so. Typically, an application for an extension will be supported by documentary evidence: however, where it is not possible for you to provide evidence please contact your Unit Coordinator.
 
The Unit Coordinator must notify you of the outcome of an extension request within 3 working days of receiving the request.
Late penalties
Assignments submitted after the deadline will receive a late penalty of 5% of the original available mark for each calendar day (or part day) that the assignment is late. Late submissions will not be accepted more than 10 calendar days after the due date, or after assignments have been returned to other students on a scheduled date, whichever occurs first. Further information on Late Penalties can be found on the Assessments and Results Procedure.
 
Review of results and appeals
You are entitled to ask for a review of the marking and grading of your assessment task if there is an irregularity in the marking standards or an error in the process for determining the outcome of an assessment. Details on how to request a review of a mark for an assignment are outlined in the Review and Appeal of Academic Decisions Procedure.
