| | | | | | | | | | | | | | | Assessment Task 1: Diagnostics A3 poster, issues log and project documentation (ACI-NSW Cohort only) | Task Description: | This assessment task is to be completed by ACI-NSW Special programs students only and is the sole assessment piece for this cohort. This assessment task fulfills the unit requirements for students undertaking CAM509 in partnership with the Agency for Clinical Innovation at NSW Health. There are three components of this assessment:
Diagnostics poster
The A3 poster’s purpose is to clearly convey the purpose of your project, the findings from your diagnostics phase, and your case for change. There should be clear, logical linkages between your project goal, the evidence and data presented, the identified root causes and key issues, and your case for change. Please use the templates and guidelines provided to complete your A3.
Issues log The issues log should contain all the issues identified throughout the diagnostics phase. Please include a source (i.e., through which tool or activity they were identified), issue title, issue description and organise them into categories.
Project documentation
You will also need to submit your supporting project documentation including your communication plan, risks and issues register and Gantt chart. The project documentation should demonstrate detailed planning for the diagnostics phase and forward planning for the upcoming solutions phase. Please use the templates provided.
